The references must have known the student for at least one (1) year and should represent at least three of the following: education\, employment\, community involvement\, or personal. Family members should not complete the reference forms.\nPersonal support inventory completed by parent/guardian\nStudent questionnaire\nRelease & Exchange of Information form\n\nYou will also upload the following documents in the Application Portal. \n\nStudent’s most recent psychological evaluation (must list diagnosis and have information about intellectual functioning and adaptive skills)\nStudent’s most recent IEP\nStudent’s academic transcript (if applicable)\n\nYou will also be asked to pay the $45 application fee.
